It is better to complete a form even if you cannot declare Baptism/Dedication or have your form signed by a Priest , Vicar or Pastor. We use this form to rank the children and this means we can be accurate in our ranking of all the children against our Admissions Criteria. Your application will still be considered if you have not completed a form.
